Two female patients have died after allegedly being administered the wrong injection at Sher-e-Bangla Medical College Hospital in Barishal this morning (15 March), prompting the authorities to form a probe committee.
The deceased were identified as Sefali Begum, 60, wife of Mannan from Dabluganj village in Kalapara upazila, and Helena Begum, 45, wife of the late Babu Hawlader from the Kashipur Airport Police Station area in Barishal.
The incident took place at the hospital’s ear, nose and throat (ENT) department.
Confirming the matter, hospital’s Director Brig Gen Dr AKM Moshiul Munir described the incident as “deeply unfortunate”.
He also said a three-member investigation committee has been formed, headed by Prof Dr Aminul Haque.
“The authorities will take necessary action against those responsible based on the findings of the investigation,” he said, adding that the hospital would provide full cooperation if the victims’ families seek legal action.
According to family members, Sefali had been admitted with a tumour on her cheek while Helena was undergoing treatment for thyroid complications. Both were scheduled to undergo surgery.
Relatives alleged that a senior staff nurse mistakenly administered a 10mg anaesthesia injection (Nor Q) to the two patients in the morning. Within two to five minutes of receiving the injection, both patients fell critically ill and later died.
Family members have demanded punishment for those responsible for the incident.
The accused nurse, Molina Rani Haldar, said the incident was unintentional and occurred by mistake.
“It was not done deliberately. I apologise for what happened,” she said.
